Sports Physicals
Wed. May 14, and Thurs. May 15 sports physicals will be given.

Staff from the Hillsboro Family Practice Clinic will be at Hillsboro Middle School and High School to administer free physicals to all interested students (grades 6 - 11).
All high school students in PE, sports, or any activity traveling for HHS, need to have a physical. All middle school students out for a sport also need to have a physical.
Forms were mailed out last week for you and your student to fill out. Please return these to the office by May 14 or earlier. Please fill out the top portion of the physical form, be sure forms are signed on both the back page of the physical form and the two places on the Permission to Participate form, and insurance numbers are given.
If you choose NOT to participate in this free event, your physical and Permission to Participate forms need to be turned in to the High School Athletic Director by August 1, 2008.
